Do I need to include a Reimbursement Claim Form when submitting documentation for a Flex Card transaction?

No, please do not send a Reimbursement Claim Form with your Flex Card documentation. Instead, please include a copy of the Documentation Request, or simply write your name, company name, and transaction number on the documentation.

Note that there is a separate fax number for Flex Card documentation: 800-722-8382.
