Getting Started With 5500 Prep Service

1) Review our Form 5500 Service.

2) Collect applicable Schedule As, Cs, etc. from carriers and service providers.

3) Download and complete 5500 Data Gathering Form.

4) Submit to completed Data Gathering Form and Schedules to ProBenefits.

5) ProBenefits will review the Form and Schedules and provide a formal quote for services.*

6) Upon approval from client, ProBenefits will complete the forms and provide a link to a portal where the authorized EFAST2 user will log-on, sign the form, and submit the form(s) electronically.

*The standard fee is $200 for the first Form 5500 and one Schedule A, and $100 for each additional Schedule. Example: ABC Company has 2 welfare benefit plans filing separately. The first plan is the ABC Company Group Health Plan with one insurance carrier and is filing as ERISA Plan 501. The second plan is the ABC Company Supplemental Benefit Plan and has 3 different Schedule As for Dental, Life, and Vision and is filing as ERISA Plan 502. The total cost to complete both 5500s and related schedules would be $500. This assumes that the filing is timely and all materials are provided by the company.
